Manager, Care Coaching

Reporting to the Associate Director, Care Coaching, you will support the work of Care Management staff. You will provide daily leadership and mentoring to staff. You will deliver services in a manner that expresses Humana values and the Family Care program values of access, choice, quality, and cost effectiveness. This position supports IDT staff that cover Jackson and Trempealeau Counties. Main Responsibilities: Mentor and support the work of Field Nurses and Care Coaches (or IDT) to ensure the Family Care benefit is delivered in compliance with the DHS contract. Identify learning needs; arrange for mentorship, provide clear expectations, facilitate orientation and ongoing learning to build capacity of IDT staff.
